Just paint the picture ... well, I struggled with this for probably 15 of my 20 years in hospice! I loved taking care of my patients, but I dreaded the documentation so much that there were times I questioned whether or not I wanted to stay in hospice.

This handy pocket-sized book gives the hospice nurse everything they need to easily illustrate decline and eligibility every time they document. Contained within the book are examples of documentation relevant to each LCD, with tips on what to document and what not to document. The book fits easily in your scrub or jacket pocket and the pages are laminated which allows you to easily wipe them with your favorite disinfectant.
